School Forms + Parent Information, 2024

Please note that we aim to make this as easy for all of our families as possible; however, New York State requires forms are filled out and signed by parents each year (some also by health care providers), and that we have hard-copy forms on-file at all times. Therefore, all forms must be returned in HARD COPY form by August 23. Thank you for your support!

REQUIRED Forms for ALL CHILDREN

• NY State Day Care Registration Form

• Child in Care Medical Statement (including all NYS required vaccines - COVID-19 and flu vaccines also strongly recommended in a preschool setting; health care provider signature required)

• Pick-Up Authorization + About Your Child

• Photo/Media Permission

IF YOUR CHILD HAS ANY INDIVIDUAL NEEDS

The following are required for children with needs related to allergies, health, behavorials, or special education/early intervention:

• Individual Health Care Plan Form (required for all children with IEPs, services, allergies, medical considerations or additional special needs)

• Individual Allergy and Anaphylaxis Emergency Plan and Action Card (required for all children with allergies, health care provider signature required)

• Medication Consent Form (only complete if your child requires emergency medication - inhaler, epi-pen, etc. - health care provider signature required)

• Parent Consent Form (required if your child will receive services from a therapist/specialist on-site)
